Modern orthodontics is facing new challenges due to both the complexity of clinical cases and the high expectations of patients. For high-quality orthodontic treatment, it is necessary to take into account the challenges of modern orthodontics - the duration of treatment, the adjacency with orthognathic surgery, psychological and aesthetic expectations, relapses after treatment, accessibility and cost. The main challenge is to combine efficiency, safety, aesthetics and comfort, while maintaining an individual approach to each patient. Not all orthodontists have time to adapt to this challenge and often choose “quick methods” (braces, aligners) when interdisciplinary cooperation or orthognathic surgery is actually needed. I will analyze the main key mistakes of orthodontists - underestimation of skeletal pathology, lack of complete diagnosis, camouflage the problem instead of solving it, moving teeth into the wrong position, ignoring the patient's age, and the psychological aspect.I will demonstrate my own cases of surgical intervention to correct skeletal anomalies.
